The FDA advised consumers not to buy or serve Once Upon a Farm Tractor Wheels Organic Toddler Soft-Baked Bars, citing a potentially fatal choking hazard. The agency said it has received several reports of toddlers ages 1.5 to 2.5 years experiencing complete airway obstruction and being unable to breathe after eating the product. When chewed, the bars reportedly form a dry, sticky mass that standard choking interventions, including back blows and the Heimlich maneuver, may fail to dislodge.

Some reported choking incidents required emergency response intervention and hospital observation. The FDA said the product is sold nationwide online and in stores including Costco, Target and Walmart. It may also be sold through Amazon.com and other third-party vendors. The agency said it will update its advisory as more information becomes available.
